Did you keep your kids' crib cards? For some odd reasons I really loved mine...they just make me feel very sentimental and really take me back to my birth moments.
Now let me preface this by saying I'm by no means anti-formula. I am pro-breastfeeding but every single one of my babies has had breast and formula to on extent or another (Nathan was 1/2 and 1/2, Naomi was about 95% formula fed and Parker is 95% breastfed)
That being said, while I'm sitting here catching up my pregnancy albums, I dug out my crib cards and scanned them in for their scrapbook. Back in the day when Nathan was born, it specified that they were breast or formula fed. That way they wouldn't feed the baby formula. I checked, and Nathan's crib card was made by Medela. Nowadays they just say I'm a boy or I'm a girl and the cards are made by the formula companies. Just an interesting observation.
Here are their cards (I studied them and there is nothing I don't mind sharing)
