CD/DVD Holder templates..

I just made some DVDs for my family and friends so they can view my scrap pages on tv and want to make envelopes to put them in.. i did see them somehere, had them bookmarked on the old, dead laptop lol so now i lost it...

so anyone know of one? or have a example of one?
