Hi, there! First of all - gotta say thanks for your purchase! I hope that I can help you out here...
I haven't used PSE4 itself, but I have used versions 1-3, so I don't think it's too much different. The exact terms might be a little off, if so I apologize!
Alright, so you have the file open - first step is to save the file as something different - so you don't accidentally overwrite your file!
Next, open up a paper that you'd like to use. For the smaller sized layouts (Shout-It-Out, Horizontals, etc) you might want to resize the papers first so the patterns aren't HUGE...so do that first - resize to 6x6 inches.
Then copy/paste or drag/drop (depending on how you prefer to do it) the paper into the layered Shout-It-Out template.
In the layers palette on the right side of the page, make sure that the paper layer is DIRECTLY over the layer that you want it to take the shape of. You might have to drag it around in there to line it up properly.
Now, on your keyboard, hit Ctrl+G at the same time, and the paper SHOULD take the shape of the layer below it.
Continue doing that until you've covered all the places papers should go - you can do the same for the photo as well. Spots for elements are a bit different, though - you should find one in your stash somewhere, that you'd like to use, and put it on the template, and then either hide or delete the element shape that I put on there - it's just a placeholder to show you where your elements should go.
Same thing applies for the title/journaling - using the text tool and your choice of fonts, type in your title & journaling and hide or delete my placeholder text layers.
