Birthday Candle Template

Hi All - does anyone know off hand who made the birthday candle template as shown in Melissa Bennett's kit preview pages - for her kit Candles and Confetti? I asked Melissa and it is not included in her kit - so it was either made by team member on her own - or was a template. Anyone seen this template in their travels? I've searched but no luck.... My image is not showing up... I'm doing something wrong - it's been yrs since I've done this... wow - LOL
