ahhh I need help!!! Need this like yesterday LOL

I was responsible for decorations for my daughters baby shower. I told her I was going to do a "questionnaire' for the guests to fill out that she could then put into her baby book. I know I have seen them before but need help. What kind of questions should I ask? Also if you have inspiration for this particular thing link me up please?!?! Shower date is Sept. 2nd. I still have to get the questions in order, make the questionaire, take it to be printed and cut it (since Ill try to do at least 2 per sheet). So I realllly need help!!!!
 
Questions would be like when did you find out mom and dad were expecting?

What would you tell mom and dad to be?

Stuff like that...
 
This isn't really what you're asking about, but it's an idea I LOVE!!! When my friend had my shower for DD1, she had 18 envelopes numbered 1-18 with a slip of paper for each envelope. Guests took an envelope and wrote a note for DD to read on that corresponding birthday. She is 12 and still looks forward to opening her special note each year.
 
Yay Yes Jacinda that is exactly what I was looking for. I may have even pinned that exact one. But Pinterest was being dumb and giving me error 502 so I couldnt check LOL

I did this for my brother's wedding - 50 of them!! It was a hit!
 
Amy, I revised them to say Love,______ (no actual line just a space to put their names :) ) I looked at them and thought something was missing and realized it was the closing LOL. We have cupcakes being made on Saturday (my daughter's mother-in-law works at the school cafeteria so she can bake up a big batch when the cafeteria is closed.) I bought zebra print, giraffe print and bright pink with white swirls cupcake wrappers. They are foil lined so the colors will stay vibrant. We bought some green tablecloths for the tables and we have some little blow up trees to decorate the tables as well as some little jungle beanie babies. Then the banner with the name Austyn Bailey. I can't remember everything else. I will be getting to the park early on Saturday along with a few other people to set everything up. My husband is working the grill and we have a bunch of shower games to play as well. I am really excited. It is the first baby shower I have ever thrown and my first grandchild. So double ecited there!
 
they are adorable! one thing - you should put a love, and a line at the bottom so the guests write their name on them! :)
